Medicare local coverage

Medicare LCD list by topic

Local coverage determinations (LCDs) are written by the Medicare Administrative Contractors (MACs) that process claims in each part of the country. Where there is no national coverage determination, or the NCD leaves room for local criteria, the LCD decides whether Medicare pays for a service in that jurisdiction.

This list groups 955 LCD records into 475 topics. Contractors often publish the same LCD under their own ID, so each topic page puts them side by side: which contractors have one, its status, when it was last updated, and the billing and coding article that carries the codes.
