Blue Cross Blue Shield medical policies
Blue Cross Blue Shield is not one insurer. The Blue Cross Blue Shield Association describes itself as an association of independent, locally operated Blue Cross and Blue Shield companies, and each company publishes its own medical policies, reimbursement policies and prior authorization rules.
To find the policy that applies, identify the local plan that issued the member's coverage, then go to that plan's provider site. Anthem is the Blue plan in a number of states; its policies are on the Anthem page.
